Seeing blood after bowel movements is a common symptom. The most frequent cause is hemorrhoids, but anal fissure, polyps, diverticular disease, inflammatory bowel diseases, or rarely colorectal cancer may also lead to rectal bleeding. For this reason, blood in stool should never be considered completely harmless.
Bleeding Characteristics Matter in Diagnosis

Bright red blood: Suggestive of hemorrhoids or fissure

Blood coating stool: May originate from colon or rectum

Dark/tarry appearance (melena): Suggests upper GI source

Clots: May indicate hemorrhoids, polyps, or inflammation
Most Common Causes of Rectal Bleeding
Hemorrhoids:
Characterized by bright red bleeding after defecation. Itching or a sense of fullness may accompany.
Anal Fissure:
Severe pain during defecation with minimal bright red bleeding.
Polyps and Colorectal Cancer:
May present with painless bleeding. Screening after age 45 is important.
Inflammatory Bowel Disease (IBD):
Bloody stools may occur in ulcerative colitis and Crohn disease.
