    Seeing blood after bowel movements is a common symptom. The most frequent cause is hemorrhoids, but anal fissure, polyps, diverticular disease, inflammatory bowel diseases, or rarely colorectal cancer may also lead to rectal bleeding. For this reason, blood in stool should never be considered completely harmless.

     

    Bleeding Characteristics Matter in Diagnosis

     

    • Bright red blood: Suggestive of hemorrhoids or fissure

    • Blood coating stool: May originate from colon or rectum

    • Dark/tarry appearance (melena): Suggests upper GI source

    • Clots: May indicate hemorrhoids, polyps, or inflammation

     

     

    Most Common Causes of Rectal Bleeding

     

    Hemorrhoids:

    Characterized by bright red bleeding after defecation. Itching or a sense of fullness may accompany.

     

    Anal Fissure:

    Severe pain during defecation with minimal bright red bleeding.

     

    Polyps and Colorectal Cancer:

    May present with painless bleeding. Screening after age 45 is important.

     

    Inflammatory Bowel Disease (IBD):

    Bloody stools may occur in ulcerative colitis and Crohn disease.

     

    Alarm Symptoms

    Evaluation should not be delayed if the following are present:

     

    • Age ≥ 45

    • Family history of colorectal cancer

    • Weight loss

    • Anemia

    • Nocturnal symptoms

    • Persistent bleeding

    • Abdominal pain + diarrhea

     

     

    Diagnostic Evaluation

    Assessment may include:

     

    • Anorectal examination

    • Rectoscopy

    • Colonoscopy

    • Imaging if necessary

     

     

    When to Be Concerned

    Even in young patients, recurrent bleeding requires evaluation.

     

    Treatment Approach

    Treatment depends on the underlying cause:

     

    • Hemorrhoids: Laser, THD, band ligation

    • Fissure: Botulinum toxin, sphincterotomy

    • Polyps: Endoscopic removal

    • IBD: Medical therapy

    • Cancer: Multidisciplinary management
